Please submit all health forms and authorizations prior to the start of the school year for each student.

Health Forms & Immunization Records
Required for All Students
All students are required to have signed health forms and immunizations records from their physician on file with the school before the first day.
Allergy Forms
Please complete the Allergy Care Plan form for students who have known allergic reactions.
If your child has an allergy which may require an emergency medication, we ask that you notify us immediately and that your doctor complete both the Allergy Care Plan and Emergency Allergy Plan forms linked below by August 1st.
Medication Authorizations
Section A: Temporary Authorization
–to be completed by parent / guardian–
If your child requires a temporary (10-day) prescription medication or over-the-counter medication, Section A of this form can be filled out by parents. This form, along with the medication, authorizes TMS to administer the drug for 10 days.
Section B: Authorization for >10 Days
–to be completed by physician–
If your child requires medication throughout the school year, please have your doctor complete Section B of this form.
Sunscreen / Diaper Cream Application Permission Form
This form is required for parents who would like us to apply an over-the-counter skin care product to their children.
